This course focuses on evidence-based, patient-centered care at the individual and population level in the assessment and management of older adults with multiple chronic conditions, including prevention and management of geriatric syndromes. Analysis of the healthcare delivery system to support optimal care transition, maximizing quality and function across the health continuum and care settings. Strong emphasis is placed on health promotion; disease prevention; and providing ethical and culturally competent care to underserved, marginalized, and diverse patients/populations. Extra Fee.
