The University offers a vision plan for all eligible, regular employees and their dependents (FTE .50 and above). The vision plan uses Vision Service Plan (VSP) and the VSP Signature network of providers. A detailed description of the program benefits, including limitations and exclusions, is found here or by contacting the Office of Human Resources.

Identification and Filing Claims

VSP identifies employees and their covered dependents using the employee's SPU Identification number. There are no identification cards for this plan. 

A higher level of benefits is paid by the plan when a provider is seen from within the VSP network. When seeing out-of-network or non-participating doctors, you will pay the provider in full at the time of your appointment, and later submit a claim to VSP for partial reimbursement for any .  Please contact VSP directly prior to utilizing the Claim Form for out-of-network or non-participating doctors.


Cost of Plan

The University pays the full premium for regular employees' coverage (FTE .50 and above).  This is subject to change each year, based on renewal of program contracts.

The employee rate (as paid by the University) is noted below.

Employees who elect spouse and/or children coverage pay for the entire cost of their dependents' premiums by payroll deduction.

All vision insurance premiums deducted from employee earnings are taken on a pretax basis.
